Share Your Advance Healthcare Directive With Your Medical Providers

Once you complete and sign your Advance Healthcare Directive (AHCD), be sure to upload it to your Healthcare Member Portal(s) or otherwise share it with your medical providers. Having it on file as part of your medical records will make things easier for you and your loved ones. We've included the link to Kaiser's Member Portal here and will add other provider links as we are able. ​Before uploading your AHCD to the Kaiser Permanente Portal, write your Member ID # on the first page and have your log-in credentials ready.
